I am writing to thank you all for the excellent support you have provided to GD particularly during the latter part of his care/support with your service. GD himself has confirmed this to me in our telephone conversation at the time I was ending my involvement in his support on 26th July 2012.

He confirmed to me about his fears when the initial plan was for him to move from a registered care managed by yourselves to a more independent living accommodation; but said he was encouraged and motivated by the level of encouragement, reassurances, and the consistent person centred support he received from yourselves before, during and after his move into his own flat.

We rarely see some of our learning disabilities clients moving-on from a registered care into supported living placements and then becoming fully independent in their own flat at the point where they do not require further support from care/support provider; but in this case, you have delivered an outstanding job to help GD take control and relevant risks in his own life to achieve this. Well done!

Furthermore, I would like to thank you again for the level of cooperation and openness you have shown in working together with Southwark Social Services and other relevant services to promote the needs of GD.

My Line Manager and other colleagues who have heard aboutGD’sachievement have welcomed this as a great achievement and excellent example of an ‘Outcome Base’ support.
